Heartland Community Group (HCG) works with local (Wayne County, Ohio) schools, churches, mental health and social service agencies, along with community members, to identify within the community “aged-out” individuals who may benefit from participating in a transitional housing and support services program.
An “aged-out” individual can include one who has recently ended a stay in foster care or a youth residential facility or has been referred to HCG by a local school, church, or mental health agency and demonstrates a need for assistance in housing, short-term continued care, and assistance with independent living skills.
HCG works with the participant to identify and work towards short-term goals including attending local adult-education programs, taking college courses, and securing part-time or full-time employment.
HCG assists with securing a housing unit and provides support services while the individual completes predetermined goals.
HCG connects participants to resources including friendship support within a local church, outpatient mental health services, and more!
